چکیده

Background: Empagliflozin reduces the risk of hospitalization for heart failure in patients with type 2 diabetes and cardiovascular disease. We sought to elucidate effect empagliflozin as an add-on therapy on decongestion renal function admitted acute decompensated failure. Methods: The study was terminated early due COVID-19 pandemic. enrolled 59 consecutive Patients were randomly assigned receive either (n=30) or conventional glucose-lowering (n=29). performed laboratory tests at baseline 1, 2, 3, 7 days after randomization. Percent change plasma volume between admission subsequent time points calculated using Strauss formula. Results: There no significant differences left ventricular ejection fraction serum NT-proBNP (N-terminal pro-B-type natriuretic peptide), hematocrit, creatinine levels groups. Seven randomization, level significantly lower group than ( P =0.040), hemoconcentration (≥3% absolute increase hematocrit) more frequently observed =0.020). decrease percent larger randomization =0.017). incidence worsening (an ≥0.3 mg/dL) did not differ Conclusions: In this exploratory analysis, achieved effective without increased Registration: URL: https://www.umin.ac.jp/ctr/index.htm ; Unique identifier: UMIN000026315.

Heart Failure Potential Effects of Aggressive Decongestion During the Treatment of Decompensated Heart Failure on Renal Function and Survival

Background—Overly aggressive diuresis leading to intravascular volume depletion has been proposed as a cause for worsening renal function during the treatment of decompensated heart failure. If diuresis occurs at a rate greater than extravascular fluid can refill the intravascular space, the concentration of such intravascular substances as hemoglobin and plasma proteins increases. We hypothesi...

Effect of Bromocriptine Mesylate as Add-On Therapy in Obese Type 2 Diabetes Mellitus Patients

Six months administration of bromocriptine mesylate significantly decreased glycated hemoglobin (HbA1c), fasting blood sugar, postprandial blood sugar, and weight of 22 Indian obese patients with type 2 diabetes mellitus with no serious adverse events. Therefore, the novel mechanism of action, efficacy and acceptable safety profile makes this drug an attractive option for treatment of obese typ...

Effect of aminophylline on renal function of brain injury patients with acute renal failure admitted to the intensive care unit

Introdution: Acute renal failure is a common complication in the brain injury patients. Released adenosine in injury can reduce renal glomerular filtration. Aminophylline is an inhibitor of releasing adenosine. So, this study aimed to evaluate the effect of intravenous aminophylline in renal function of brain-injured patients with acute renal failure admitted to the intensive care unit. Method...

Influence of Titration of Neurohormonal Antagonists and Blood Pressure Reduction on Renal Function and Decongestion in Decompensated Heart Failure.

BACKGROUND Reduction in systolic blood pressure (SBP reduction) during the treatment of acute decompensated heart failure is strongly and independently associated with worsening renal function. Our objective was to determine whether SBP reduction or titration of oral neurohormonal antagonists during acute decompensated heart failure treatment negatively influences diuresis and decongestion. M...

Markers of decongestion, dyspnea relief, and clinical outcomes among patients hospitalized with acute heart failure.

BACKGROUND Congestion is a primary driver of symptoms in patients with acute heart failure, and relief of congestion is a critical goal of therapy. Monitoring of response to therapy through the assessment of daily weights and net fluid loss is the current standard of care, yet the relationship between commonly used markers of decongestion and both patient reported symptom relief and clinical ou...
